Civil
Supplies department provides the following additional services to family
cardholders through Assistant Commissioner's and Taluk Supply Officers :
1,
Can an applicant get a duplicate family card if it is lost ?
Yes,
the applicant can apply for a duplicate family card provided it is lost or
damaged. The documentary requirements are as below:
-
Written
application by the head of family indicating the reasons for applying for
a duplicate family card, the ration card number and FPS code.
(Applications from any other member of the family will be rejected.), AND
-
Ration
card number or xerox copy of the lost family card OR the damaged family
card and name of the fair price shop, AND
-
Proof
of residence in the address in the ration card, AND
-
Proof
of current residence (as for a new family card)
2.
Can a new family member be added to an existing family card ?
Only
close relatives of the cardholder may be added to an existing family card,
provided the member has shifted permanently to the house of the existing
cardholder or in case of a newborn child. Documentary requirements are as below:
-
Written
application (in white paper) by the head of the family of the existing
cardholder, with full address, name, previous address of the new member of
the family, nature of relationship with the head of the family, AND
-
Certificate
of deletion from the family card of parent or guardian in which his /her
name was included earlier (OR)
-
Certificate
of non-inclusion of his / her name in any family card in the previous
address. (OR)
-
Birth
certificate of the child, in case of a new born child of any existing
member in the family card.
3.
How do applicants apply for deletion of a family member from the family card?
Members
in a family card may be deleted on application by the head of the family owing
to valid reasons, such as shifting away to another separate and distinct
residence within or outside Tamil Nadu. Documentary requirements are as below:
-
Written
application (in white paper) by the head of the family of the existing
cardholder, with full address, name, previous address of the new member of
the family, nature of relationship with the head of the family, (OR)
-
Written
application from any adult member in the family card who has moved away to
any other distinct and separate house, with reasons for such movement AND
-
Proof
of new separate and distinct residence of the member of family, (OR)
-
Proof
of reason for moving permanently to any other State or country.
4.
How can change of address be recorded in the family card ?
-
In
case the applicant moves into another residence within the jurisdiction of
the same Fair Price Shop, the change of address can be recorded in the
ration card following the procedure and documentary requirements below.:
-
Written
application from the head of the family indicating the reason for
change in address and the new address AND
-
Proof
of new residence (copy of any one document as for a new card) AND
-
Original
ration card
-
Once
these documents are submitted, an acknowledgement slip is given with
a time limit as in the Charter, the changes are carried out in the
family card and the applicant can come and collect the updated
ration card on the day stipulated.
-
In
case the applicant moves into another residence outside the jurisdiction
of the same Fair Price Shop, the change of address can be recorded in the
ration card following the procedure and documentary requirements below.:
-
Written
application from the head of the family indicating the reason for change
in address and the new address AND
-
Proof
of new residence (copy of any one document as for a new card) AND
-
Original
ration card
-
Once
these documents are submitted, the following procedure is followed:
-
a
deletion slip is given to the applicant who the has to go to the Fair
Price Shop concerned, get the card deleted from the A-Register and get a
deletion slip signed by the salesman
-
Hand
over the deletion slip with the family card to the AC/TSOoffice and get
an acknowledgement slip with a time limit as in the Charter,
-
The
changes are thereafter carried out in the family card by AC/TSO and
-
Applicant
can come and collect the updated ration card on the day stipulated.
5.
Issue of Surrender Certificate on transfer of family to other City or State or
otherwise
In
case the applicant moves into another city outside the jurisdiction of the taluk,
the applicant is issued a separate surrender certificate following the procedure
and documentary requirements below.:
-
Written
application from the head of the family indicating the reason for change
in address and the new address (if any) AND
-
Original
ration card
-
Once
these documents are submitted, the following procedure is followed:
-
a
deletion slip is given to the applicant who the has to go to the Fair
Price Shop concerned, get the card deleted from the A-Register and get a
deletion slip signed by the salesman
-
Hand
over the deletion slip with the family card to the AC/TSOoffice and get
an acknowledgement slip with a time limit as in the Charter,
-
Changes
are thereafter carried out in the family card by AC/TSO and a surrender
certificate prepared.
-
Applicant
can come and collect the updated ration card on the day stipulated with
the surrender certificate in case the applicant is moving to any other
taluk within Tamil Nadu.
-
In
case, the applicant is moving to any other State, the ration card is
cancelled and retained for destruction by the AC/TSO office. The
applicant is given the surrender certificate on collection of the
acknowledgement slip.
6.
If an applicant does not want a family card, can a No Card certificate be
issued?
It
is the choice of every family to decide whether it wants a family card. In case,
the family does not want to avail of commodities, it is possible to obtain a
no-card certificate OR a No-commodity card (white). In case the applicant
chooses to apply for a No-card certificate, then the following documentary
requirements are to be met:
-
Application
by the head of family (in the same format as the new card application)
in the application
format in English or application
format in Tamil with all documents as applicable for a new card
and with a self-certifcation that the family does not possess any family
card anywhere in India.
-
The
same procedure of inspection will be followed and order will be passed
by the AC/TSO.
-
This
certificate will be valid for a period of 1 year and will be recorded as
part of the A-Register for the FPS having jurisdiction over the
residence of the applicant.
-
The
applicant will not be eligible for applying for a family card for a
period of 1 year till expiry of the NCC.
7.
Can a family not possessing a ration card get a No-Objection Certificate for
getting ration card in another State?
In
case the applicant without a family card in Tamil Nadu moves with family to
another State, the applicant can apply for a No-objection certificate for
applying for a family card in that State. The procedure and documentary
requirements are below:
-
Written
application from the head of the family indicating the reason for
shifting to another State and the new address (if any) AND
-
Proof
of present residence in Tamil Nadu
-
Once
these documents are submitted to the AC/TSO office, the following
procedure is followed:
-
An
acknowledgement slip with a time limit as in the Charter is issued to
applicant
-
The
application is verified through field inspection (if necessary) and
taluk A-Registers are checked as to whether the family is already in
possession of a famil ycard in TN.
-
If
the verification reveals that the applicant has no family card, a NOC is
prepared.
-
Applicant
can come and collect the NOC on the day stipulated after surrendering
the acknowledgement slip.
8.How
can a cardholder apply for inclusion under Antyodaya Anna Yojana Scheme ?
-
Under
the Anthyodaya Anna Yojana Scheme, the Government of India has ordered,
based on Hon'ble Supreme Court of India directions, that the poorest of
poor shall be supplied 35 kg of rice to ensure that they are able to ward
off hunger and malnutrition. The following eligibility conditions need to
be satisfied for selection under AAY.
-
An
applicant has to apply in the AAY application format to the AC or TSO
concerned and satisfy the following conditions:
-
The
family must have a green colored all commodity card
-
The
family must be from the poorest of poor segment of society
-
The
family must be from one of the categories specified by Government of
India in its circular on AAY eligibility norms.
-
Preference
will be given for SC/ST families who are below poverty line
-
Applicants
may collect and acknowledgment slip with date of application and serial
number from the AC/TSO office
-
Applications
will be processed as below:
-
The
AC/TSO concerned will ascertain the BPL status of the applicant from the
DRDA.
-
Thereafter,
after field inspection, the AC/TSO will satisfy himself about the
eligibility of the applicant under AAY. Thereafter, the application is
sanctioned.
-
After
sanction, an order is issued and the applicant is informed by post card
to appear with the ration card. This process has to be completed within
60 days from the date of application.
-
On
production of the family card, AAY stamp is affixed on the card and
necessary entries are made in the taluk A- registers.
-
The
AC/TSO shall also send an AAY inclusion slip along to the FPS through
the applicant for carrying out changes in the FPS A-Register.
-
In
case the application is rejected, the AC/TSO shall send a post card to
the applicant indicating reasons for rejection
9.
How to apply for a Free LPG connection and LPG Stove
-
Government
of Tamil Nadu has obtained sanctions for 9 lakh LPG connections for
release for BPL families. This scheme is not a permanent scheme and will
be open till the connections under the scheme is exhausted. However,
Government of TN has requested for another 8 lakh connections in 2008-09.
-
This
scheme makes provision for supply of a free LPG connection along with a
free LPG stove. The applicant has to pay for the refill deposit and all
cylinder costs thereafter. LPG connections and stove are sanctioned in the
name of the adult woman member of the family.
-
In
order to obtain a free LPG connection and free LPG stove, an applicant has
to satisfy the following eligibility conditions:
-
The
applicant family should not be in possession of an LPG connection
till date
-
The
family has to be below poverty line
-
The
family must live in a house that satisfies the safety norms laid
down by the Oil industry for sanction of LPG connections
-
The
applicant must be willing to pay the cylinder deposit and the cost
of refille thereafter.
-
Applicants
have to apply to the AC/TSO concerned and the following procedure will be
adopted for sanction of a connection:
-
Written
application by head of family with family card details, present address,
-
All
applications are send for field verification by a team comprising of the
local councilor (urban areas) or Panchayat President (villages), VAO,
etc. and their recommendations obtained
-
After
shortlisting, the LPG dealer inspects the houses from the safety point
of view and thereafter approves eligible houses.
-
After
TNCSC releases the deposit towards the LPG connection, the applicant has
to pay the cylinder deposit
-
After
the payment, the LPG dealer issues the Subscription Voucher for the
connection. after which the connection is released.
-
TNCSC
thereafter arranges to have the LPG stove delivered to the residence of
the applicant and the LPG dealer sends the LPG cylinder.
10.
How to file a complaint on a grievance in a AC/TSO office or Fair Price Shop ?
Any
applicant or cardholder may file a complaint through the following means:
-
Deputy
Commissioner or District Supply and Consumer Protection Officer of the
district concerned
-
District
Collector in person or through Monday Grievances day or fax
-
Online
grievance redressal system: This system enables to register a
complaint about services provided by a AC/TSO office.
